Immunization clinic for rising 7th and 12th graders the week of April 24th -28th

In conjunction with the Yadkin County Health Department, Yadkin County Schools will provide an immunization clinic for rising 7th and 12th graders the week of April 24 th -28 th . Staff from the Yadkin County Medical Clinic will be at each middle and high school to give immunizations to all families that opt-in. If you want to use this service, please complete our online application
process using the links below. These links are also on the Yadkin County Schools homepage in the Latest News section. All online consent forms can be completed on a phone, tablet, or computer and should be submitted by Friday, March 31 st . If you need help completing the form online, you can contact your school nurse for a paper copy of the application. Your school will let you know their specific immunization day after spring break.
